In-Home Community Support

At Equalworks, we believe every person deserves the opportunity to thrive in their own home and community. In-Home and Community Support Services are designed to promote independence, dignity, and inclusion for individuals with intellectual disabilities or autism. Our services are rooted in the principles of person-centered care, ensuring each individual’s rights, preferences, and goals come first.

What Is In-Home Community Support?

This service provides individualized assistance with:

• Daily living skills (e.g., cooking, cleaning, budgeting)
• Personal hygiene and self-care
• Community participation and safety
• Social and relationship-building skills
• Communication and advocacy support

Services are delivered in the individual’s home or community and are tailored to support their unique abilities and personal goals.

Eligibility

To qualify, individuals must:

• Be 18 years or older
• Be enrolled in a Pennsylvania waiver program (e.g., Consolidated, Community Living, or P/FDS Waiver)
• Have a qualifying diagnosis of an intellectual disability or autism
• Have a service plan identifying In-Home Community Supports as a needed service
